Medical Virtual Assistant

During my training in Pro VA as a Medical Virtual Assistant, my mentor taught me ways on how to make a Patient's Medical History Form and an Online Doctor's Appointment Form by using Google Sheets, PDF and Google Form.
1. Google Sheets Template (for easy tracking).
2. Google Form (for easy user entry). A Google Form would allow users to fill out their appointment request easily and have the data automatically collected in Google Sheets.
3. PDF Form (fillable). A fillable PDF form where users can enter their information.
