This form records essential details, including personal information, contact details, medical history, and preferred appointment schedules to ensure a smooth and efficient booking process. In this assignment, five individuals Jenny Rose Navarro, Kristel Jade Marie Juanite, Mary Rosary Gania, Presneil Maglasang, and Mary Joy Lim have scheduled doctor check-ups in different departments. Most have chosen General Medicine (Internal Medicine), while Presneil Maglasang and Mary Joy Lim opted for Cardiology. The appointment dates range from February 28, 2025, to March 24, 2025. Some of the patients have previously applied to the facility, while others are visiting for the first time. This structured approach ensures that each patient receives timely and appropriate medical care.
