As part of this medical virtual assistant task, a patient history form is being created to serve as a vital tool for physicians in providing informed and effective patient care by documenting their medical history.

Leveraging the intuitive design tools offered by Canva, I have developed a comprehensive prescription pad intended to enhance the efficiency and clarity of medication prescriptions for physicians. Furthermore, the layout prioritizes ease of use for the physician, aiming to minimize errors and streamline the prescription writing process. By utilizing Canva, I was able to create a visually organized and professionally formatted prescription pad that serves as a valuable tool in supporting accurate and effective medicine prescription for patient care. This design also considered potential integration with electronic health record (EHR) systems by ensuring legibility and adherence to standard prescription formats.

Patient tracking in this system involves the use of the MINIMUM and MAXIMUM formulas to easily determine the earliest and latest admission times, as well as the oldest and youngest patient ages based on birthdates, offering a simple yet effective way to monitor these key patient characteristics.
