During my tenure at Medicard, one of the largest HMOs in the Philippines, I served as a junior Care Customer Officer. A significant aspect of my role involved assisting members seeking consultations or check-ups. This form was provided to members as their payment record. To facilitate its processing, I diligently collected accurate information such as their name, age, birthday, member code, chief complaint, chosen facility, and preferred doctor. Ensuring the precision of each detail, particularly their chief complaint, was crucial as it aided in directing them to the appropriate physician. Subsequently, I awaited confirmation from our internal departments, who would email me back with this attached file.
Displayed here is a sample of what we refer to as an LOA (Letter of Authorization). This document acts as a permission letter provided to our members when they require a diagnostic test. The form functions as their proof of payment. To initiate the process, members are required to email us regarding their upcoming appointment along with their personal information. Once I receive their accurate details, I'll promptly forward them to the appropriate department for processing and await their response, which will include this document attached.
