Thank you for scheduling your follow-up consultation with Dr. Reyes. This 30-minute session is intended to review your current health status, discuss lab results, adjust medications, or answer any ongoing concerns.

What You’ll Need:
✔️ Recent lab results (if applicable)
✔️ List of current medications
✔️ Notes or questions you’d like to discuss

Availability:
Monday to Friday – 8:00 AM to 4:00 PM (Philippine Time)

Booking Confirmation:
Once booked, you'll receive:

✅ Calendar invite
✅ Reminder 24 hours before the appointment

Rescheduling Policy:
If you need to change or cancel your appointment, please do so at least 24 hours in advance using the reschedule link in your confirmation email.
