Here are some of my sample works and I'm using Microsoft Word and Google Forms to create my works.
- MEDICAL HISTORY FORM (Microsoft Word) -
is a document used by healthcare providers to collect important information about a patient’s past and present health conditions, lifestyle, and family medical history. This form helps doctors and medical professionals understand the patient's overall health status and make informed decisions about diagnosis, treatment, and preventive care.
- in this Medical History Form, the medical practitioner can see essential details about the patient's past and present health, including any chronic conditions, symptoms, allergies, and family medical history. This information enables the practitioner to make informed decisions about diagnosis, treatment options, and any potential risks, ensuring personalized and safe care for the patient.
- ONLINE DOCTOR APPOINTMENT (Google Forms) -
is a digital form created using Google Forms, where patients can submit their details to schedule a virtual consultation with a doctor. The form typically collects information such as the patient's name, contact information, medical history, reason for the visit, preferred appointment time, and any other relevant details. This allows healthcare providers to review the information, confirm the appointment, and conduct the consultation online through video or phone call.
- In an Online Doctor Appointment, the medical practitioner can see essential patient information such as personal details, medical history, symptoms, and the reason for the appointment. This allows the practitioner to prepare for the consultation, understand the patient’s concerns, and offer informed medical advice. The form may also include preferences for appointment times and any specific questions the patient wants to address, ensuring a smooth and efficient virtual consultation.
