(EMERGENCY ROOM HEALTH CLERK)

- Receives and transmits telephone communications.
- Answers paging system call for doctors and nurse present in the unit.
-Transmits simple referrals to attending physicians and resident physicians.
- Receives and delivers accurate messages to and from other departments.
- Directs newly admitted patients, visitors, attending physicians and hospital personnel to patient’s
room.
-Handle’s outgoing and incoming communication on the nursing unit concisely and accurately.-
-Transcribe orders under supervision of the charge nurse.
-Works in partnership with charge nurse to provide accurate and prompt carrying out of doctor’s
orders
 Systematically transcribes orders from doctors order sheet to cardex, medicine/IVF ticket &
medication record.
 Processes requisitions and forms as directed by the charge nurse.
 Writes requisitions for: pharmacy, dietary, central service, laboratory, xray and other hospital
departments.
-Assits staff nurse with patients records.
- Fills in headings of all forms in the chart using BLOCK LETTERS.
- Keep chart in order and in proper sequence at all times.
- Attaches correct records and reports to appropriate charts and signs posted results after.
- Checks records of patients to make sure that all forms are complete and properly filled up.
- Prepares and accomplishes patients to make patients clearance before discharge.
-Check chart for all procedures performed for patient upon order off “MAY GO HOME”.
- Notifies consultants involved in the mgmt of patient upon order of attending physician for
discharge.
- Calls and reports to business office patients with discharge oder for clearance.

- Instructs patients/family member to settle bill at the cashiers office after 30 minutes of reporting
the discharge order to the business office.
- Prepares discharge instructions.
- Other added assignment (according to the discretion of the charge nurse).
- Helps in preparing vacant rooms.
- Care of equipments/ items in the station.
- Requisition of suppliers for nurses station usage.
- After care of items, sorroundings and inside the nurses station.
- Performs othe functions and responsibilities which may be assigned by the immediate superior.
