           This project demonstrates my ability to design a professional and functional healthcare document using Microsoft Word and Canva. The goal was to create a clean, easy-to-use medical history form  suitable for general patients and clinic use. The form includes essential sections such as patient information, medical conditions, medications, and family history, all structured for clarity and accessibility. The layout follows a modern, minimal design using blue and gray accents to convey trust and professionalism—ideal for use in medical or wellness settings.

                The online doctor appointment system was developed using Google Forms to simplify the process of booking medical consultations. Patients can easily provide their details, choose a preferred date and time, and submit their appointment requests online. The collected data is automatically organized in Google Sheets, allowing doctors to review and confirm appointments efficiently. This system reduces manual work, minimizes scheduling errors, and offers a cost-effective solution for managing online consultations.
