I perform swab tests on patients prior to their admission to the hospital. This is done as a protocol to prevent the spread of COVID-19. I collect the following information from the patients:

• Name
• Age
• Contact information
• Date of admission
• Symptoms
• Travel history
• Recent contact with someone who has COVID-19

I also have the patients fill out a COVID-Information Form. This form contains the same information as the above, but it also asks about the patients' vaccination status and any underlying health conditions.

I then transfer all of this information to an Excel spreadsheet. This allows me to easily track the patients' test results and other data. I can also use the spreadsheet to generate reports on a monthly basis.
