Accountable for appropriate drug therapy monitoring by reviewing each patient chart. Accurately check, double check, and verify medication orders and ensure that the patient's nurse receives the medication(s) in a timely manner. Document clinical interventions to every drug interactions, and provide recommendations.
Dispense medications and provide product information to customers according to their proper usage, and timing. Manage stocks, secure proper storage of medications, procure new supplies of medications. Monitor stocks to avoid over stock, expiry or shortage.
Review patient data to determine the appropriateness of the medication therapy. Evaluate patient's condition to ensure all issues are treated. Develop effective medication plans that minimize the risk of adverse drug reactions. Consult proper dosages and medications, advise correct administration on drugs, assess results of medication therapy, and document accurate medication plans and patient progress.
