CONTACT TRACER
*Collaborate with other government agencies and private sector for the conduct of enhanced contact tracing.
*Conduct case interviews, profiling, and perform an initial public health risk assessment of COVID-19 cases and their identified close contacts.
*Collecting and recording accurate information using CRM software.
*Assist in collecting laboratory samples and provide health education and give instructions with regards to DOH protocols.
*Refer the close contacts to isolation facilities, as necessary.
*Elevate and refer challenging or complex cases or incidents and outbreaks of COVID-19 to the designated Team Leader.
*Conduct daily monitoring of close and general contacts for at least 14 days.
*Profiling of master list for Covid-19 vaccine and encoding of reports in authorized application.
*Assist in vaccination program conducted by the Local Health Department.
*Assist government agencies and private sector in providing financial and food assistance to those families who's affected with Covid-19 virus.
COMPANY NURSE / ADMINISTRATIVE ASSISTANT
*Performs administrative support functions such as data entry, typing, filling, reviewing and processing paperwork.
*Manages the office clinic, medical records, supplies, instrument and equipment and sanitation of the clinic.
*Strictly monitor and prepare reports pertaining to all work accidents, death or occupational illnesses in places of employment.
*Perform other clerical and administrative support as assigned by superior.
