I created these  Google Forms as a part of my medical virtual assistant task.

This involves listing down questionnaires regarding the patient's medical condition which will then be the basis of the healthcare provider for the quick decision-making and proper treatment required. This simplified way of answering the sheet is also beneficial for the patient as he/she doesn't need to write any information required unless not mentioned in the form which improves patient-doctor communication.
